I think classifying it is an illness is just letting these people down easy. They aren't mentally ill. Mentally ill is autism, bipolar, HPPD, acute anxiety, etc.

I've struggled with low self esteem, depression, etc my entire life and never once did it cross my mind to destroy my body with plastic surgery lol.

Or maybe I'm unintentionally ignorant because I don't understand, I'm not sure.

Body Dismorphic Disorder is very much a mental illness, and a very serious one too. It's often the root cause behind extreme cases of anorexia, where someone who weights 60lbs looks in the mirror and perceives an overweight person.
