The name attached was one of their nurse practitioners. IMO, it was a pissed off former employee getting revenge or their password was just weak and someone decided to harass them. 

Sure, that's possible. Lots of things are possible and if something strange like that is what happened then I'm sure it'll be known. Until then, the simplest assumption is of course that there was an idiot who works there who sent the message.
